Dear sir, 
          
        Sub: Accident report
               Ref: Your letter no.2204 dated 19th sept 2010,
with reference to your letter, Iam submitting the following report after investigating the causes,extent of damage,and the condition of the victims who were injured in the accident on 19.9.10.
The accident was caused by a truck which has been used for moving the goods to godown. Mr. Ram, the truck driver, had a fracture at his left leg and was admitted in a nearby hospital. His fracture was severe that he had a knee-dislocation. The losses have been estimated to about 1.25 lakhs. The reasons for the accident are as follows:


   1. There was a slippery area on our godown which has steep slopes for the truck to enter. Rain caused the area to be slippery and so that the truck could not be at its control.
   2. As the truck is entering into the godown, heavily loaded with goods, it was unable to get to the other end of the slope. As the driver had increased the acceleration the wheels skidded suddenly due to slipping surface and fell bottom-wise in the nearby lane where the raw materials have been kept. 
   3. As all the goods in the truck got opened up, there was a greasy surface and so there can't be any access to the truck for the rescue.

               To avoid these kinds of accidents the slope should be reduced in height and the entering should be made larger. To avoid the slipping surface on the slope due to rain, there should be a covering so as to avoid the rain from falling on the slope.
The preventive measures will cost about 20 thousand rupees so as to avoid further accidents and immediate steps will be taken as soon as the fund is provided.

DEATH

Death is the permanent termination of all vital functions or life processes in an organism or cell. After death, the remains of an organism become part of the biogeochemical cycle. Organisms may be consumed by a predator or a scavenger and leftover organic material may then be further decomposed by detritivores, organisms which recycle detritus, returning it to the environment for reuse in the food chain.
One of the challenges in defining death is in distinguishing it from life. Death would seem to refer to either the moment at which life ends, or when the state that follows life begins. However, determining when death has occurred requires drawing precise conceptual boundaries between life and death. This is problematic, however, because there is little consensus over how to define life. The nature of death has for millennia been a central concern of the world's religious traditions and of philosophical inquiry. Many religions maintain faith in either some kind of afterlife, reincarnation, or resurrection.
